Facilitating a New Doctor Visit
You just moved and are establishing care with new doctors. Unfortunately, your records from your prior doctors didn’t get sent to your new doctors in time. Don’t you wish that there was a way to facilitate these visits so you don’t have to tell your medical history again and again for each new doctor visit? Below is a free download of a template that you can print and fill out (and keep current) so that you can take it to every new doctor visit so that you don’t have to tell your story again and again. Just provide a copy to each of your new doctors (or their medical assistants) and say “This is a copy of my medical history.” This way, they won’t have to ask you questions about any of this information. With appointment time with doctors being shorter and shorter, you would want each minute with your doctors to be spent with them answering your questions rather than them asking you questions you have been asked a million times.
